Must be submitted to the National Headquarters no later than 30 days after chapter conducts expulsion vote. You must include: Minutes from meeting at which this action was discussed and approved, a complete and accurate vote tally which must be included in minutes. Failure to submit proper documentation will result in delays and potential denial of Member Expulsion Form. If denied, chapter must resubmit all documentation. Minutes can be emailed to firstname.lastname@example.org.
